Individualized health care is provided to residents of Nome and surrounding villages on an outpatient basis. The multidisciplinary team works to supply the highest level of care and to promote wellness. Services include:
- Evaluation & Treatment by appointment and walk-in basis
- Pediatric Case Management
- Adult Case Management
- Procedures Case Management
- Specialty Clinic Case Management
- SART (Sexual Assault Response Team) exams and case management
- Maternal Child Health Case Management
At your appointment, patients are encouraged to do the following:
- Patients are encouraged to bring all their medications with them to their appointment.
- Patient should inform the provider if they have any allergies.
- Patients are encouraged to bring insurance information, and proof of beneficiary status when you register.
- Patients are encouraged to contact the Patient Advocate if they feel they will need assistance during their health visit.
